Pretty damn sure this is just a maid cafe. For "Otaload" read "Ota Road", the road through the middle of Denden Town. Just go to Bar 78 instead.

In Conclusion
I found another place that looked like an otaku bar, but when I maps'd it, it turned out to be what's loosely defined as a "Concept Cafe" (ConCafe), which is basically a themed bar (usually an expensive one, at that!). This is apparently one more "themed after anime" than an otaku bar proper. Though I guess there's some overlap. Also apparently there's another "BB" somewhere else.
