Review
Has a review that suggests they ignore newbies and shame customers they don't like on Social Media. I looked into it further and they have a 41-minute Youtube video explaining the "rules" of the store. They also give you a questionnaire when you arrive that you have to fill out in a manner that proves you are hardcore enough for them, or they'll kick you out. I did plan to "gaijin smash it anyway", but fack 'em. Just look at the pictures for an idea.
Getting There
In Juso. The reviewers from the Japanese site Otaku-Ba! tried to make the best of a bad job with this one, but even thier review says more about a nearby coffee shop than the actual bar.
Details
3000 yen cover charge for up to the first two hours (even if you only stay 10 minutes), rising to 4000 after 3 hours, then incrementally up to 5000 for 5 hours. All you can drink.

Well, I'll say this for 'em, they've gone to the effort of putting down a decent cyber-footprint, and making sure you know the prices before you go, and not when you get handed the bill!
